The following is a list of our approved insurance carriers: 

Blue Cross Blue Shield PPO

If your insurance provider is not listed on this page, payment is due at the time the services are rendered, and you will be responsible for filing with your insurance company. We accept cash only.

POLICY : Must cancel within 48 hours prior to appointment in order not to be billed.  You will be billed 1/3 of set fee.
